Works for me and for other users, so I suggested:
1. Unchecking the "Start at login" checkbox, then clicking "Apply", then rechecking it and clicking "Apply"?
2. Uninstalling and then reinstalling Synergy.
The user said this didn't work for him, so I wrote back saying:
Ok, can you set Synergy to launch at login and then send me the following file?
~/Library/Preferences/loginwindow.plist
I'll take a look at that file to make sure that Synergy is appears in the login items as it should.
Also, is anything printed to the console when you login? (see /Applications/Utilities/Console.app)
Finally, can you let me know where Synergy is installed and what permissions it has? These two commands show the contents of the two possible install directories:
ls -laF /Library/PreferencePanes/ ls -laF ~/Library/PreferencePanes/
On my system the latter produces output like this:

He then wrote back to me and said:
"I erased my system cache and tossed a few system prefs and performed cron maintenance scripts, restarted and synergy started automatically. I should have tried one thing at a time and given you a report but I was becoming frustrated and since I was apparently the only one having the problem, I zapped anything that I could think of (including PRAM) until something worked. I apologize for not being able to tell you what worked but hopefully mine was the only system that behaved this way."
So can you do some investigating and report back here to let us know the results?

Paul, perhaps you could read comment #2 and report more information. The problem, if there is one, can only be fixed if we actually know the details of what's happening.

Synergy does not launch automatically on system startup/login. I manually added it to the startup items, but still doesn't launch.

The most common reasons why Synergy will not launch at login include the following:
a. You might not be using the latest version
b. Your "login items" might be out-of-date, and pointing at an older version of Synergy
c. You might have installed a new version of Synergy, but you left the old version on the system as well.
So, the way to get Synergy to launch at login is therefore:
1. Ensure you have installed the latest version of Synergy. Make sure you have not left any old copies of the software in another location on your system (for example, in /Library/PreferencePanes/ or ~/Library/PreferencePanes/).
2. In the Synergy preferences, un-check the "Launch at login" preference and click "Apply", then re-check it and click "Apply". This forces any old or stale entries in your login items to be updated.
