This is not a bug but a feature. Of album art is not available in iTunes and Synergy has not yet found the art by searching at Amazon then it will show the question mark.
If it is successful in finding and downloading the art from Amazon then you will see the art the next time you play the track. Note that there may be some delay before the downloading finishes because Synergy places downloads in a queue and only downloads them after an interval of time has passed to minimize the number of requests sent to the Amazon servers.
You should be able to try some other tracks in your library and you'll see that those which have art in iTunes do indeed show art in the Floater, likewise for those for which cover art has been downloaded. If you want to see the downloaded art in the Finder, navigate to "~/Library/Application Support/Synergy/ Album Covers/".
Make sure that you are really running 1.8.1 and that you don't have an older version on your system somewhere (have a look in ~/Library/PreferencePanes/ and /Library/PreferencePanes/).
