Bug #1964: Make Command-T use Vim's autoload mechanism

Description

This should be a trivial change to make, but should probably be made to coincide with a major release, because for people using non-Pathogen installs (eg. Vimball installs) they will have to do some manual clean-up; ie. otherwise they'll have the old plug-in file at plugin/command-t.vim and the new one at autoload/command-t.vim.
